You use relfixives when the subject and the object are the same.

The childen worried us.Us is the same as we, so you would conjugate it in the fourth box, then conjugate worried accordingly. Since worried is past tense, you conjugate using the preterite tense sixth box. Los niños nos preocuparon.
Just like with verbs to describe how you feel, you use the third and sixth box to conjugate the verbs in the sentences.
They bathe themselves. They is in the sixth box, and themselves is plural, so you use -an since it is an -ar verb and in the present tense. Se bañan.
She surprised herself. She is in the third box, and herself is singular, so surprised would be conjugated in the third box. Ella surprende.
